         <title>Investigation Reveals Disabled Safety Device Resulted in Advertising Exec&apos;s Death</title>
         <description><![CDATA[<p>As an advertising executive stepped into an elevator, it started rising with the doors still open pinning half of the woman's body in the elevator shaft between the first and second floors. She was pronounced dead at the scene. <br />
A two-month investigation concluded that the accident was caused by a bypassed safety circuit that mechanics had disabled during earlier maintenance.  Video footage showed elevator mechanics leaving the building a minute before the fatal accident.</p>
